New Patient Information
The Northwestern Medicine Center for Fertility and Reproductive Medicine team provides patients with the necessary new patient consent forms and additional information about appointments to help make your fertility appointment as convenient as possible.
Please fill out our required new patient forms (New Patient Health History Form and New Patient Preoperative History Form) and return them to our office one week prior to your appointment date. You can return the forms to us by:

Appointment Information
What can I expect at my first appointment? Prior to your first visit, we will mail you a welcome packet, which includes information about our practice, medical release forms, and a health history questionnaire. We request that you complete the questionnaire prior to your first visit and bring the completed form with you. Much of the first visit will be spent reviewing your past medical history. Therefore, it is beneficial to have this information prior to your visit. Medical records can be faxed to us at 312.695.7269 or you can bring them with you. If you are being evaluated for fertility, it may be beneficial to bring your partner (if applicable) with you so relevant history can be obtained from both of you. A physical exam and ultrasound may be performed during your first visit. Your evaluation will only include tests that are relevant to your (and your partner’s) history; this will save you time and expense. You will also have the opportunity to meet with one of our highly trained financial counselors who will help you navigate the costs, insurance coverage, and payment options.
