Preimplantation Genetic Screening (PGS) or Diagnosis (PGD) PGS is used to verify that the number of chromosomes in an embryo is correct, while PGD is ordinarily used to test embryos for specific single gene mutations. Informed Consent for Oocyte Cryopreservation This consent reviews the process for oocyte cryopreservation, including the risks that this treatment might pose to you and your future offspring. Oocyte Cryopreservation Consent - Signature Page Only This includes instructions for disposition of cryopreserved oocytes. Consent for Oocyte Thaw/Embryo Transfer This includes consent for the following procedures: oocyte thaw transfers, zygote thaw transfers, embryo thaw transfers, and blast thaw transfers. |
